Name of the Jutsu :
Type : (Consists of Ninjutsu - Mainly Elemental Usage, Taijutsu - Hand Fighting Technique, Genjutsu - Illusion, Doujutsu - Eyes such as the Sharingan or Byakugan)
Rank : (For ninja in villages it is Academy Student, Genin, Chuunin, Jounin, Anbu, Sannin/Kage). (Criminal ninja use E Rank, D Rank, C Rank, B Rank, A Rank, S Rank).
Chakra Cost : How much the jutsu costs in chakra
Effects : These are what the jutsu does to your opponent

Skills :

Skills are abilities that the ninja has aside from jutsu, such as speed. They are required to perform certain types of jutsu.

Creating your own skills is allowed but its the same rule as for jutsu being you cannot make it overpowered it has to be balanced in strengths and weaknesses.
